Innovaccer Launches Copilots and Agents for Care Management

The newly launched copilots and agents will work all EHRs and PHM vendors to drive adoption without requiring large system overhauls

Innovaccer Inc., the leading Healthcare AI company, today announced the launch of Copilots and Agents for Care Management. The new capabilities aim to significantly reduce the administrative burden on care teams, with early results showing a 28% reduction in documentation time and a 20% increase in patient engagement.

Care managers often spend hours each day attempting to connect with patients, averaging three call attempts before successfully reaching a patient and as many as 10–12 attempts in Medicaid populations. These interactions are further burdened by the need to leave voicemails, follow up repeatedly, and document each outreach, taking an average of 20 additional minutes per patient.

Innovaccer’s AI Agents automate the initial phases of this workflow by initiating outreach through SMS and voice calls, sending reminders, handling scheduling tasks, reducing repetitive workload, and increasing efficiency. The Care Management Copilot automates high-burden tasks such as documentation, chart summarization, and care plan creation. By reducing manual effort, the solution enables care managers to spend more time engaging with patients and coordinating care, rather than navigating administrative work.

The platform also includes an ED Follow-up Agent designed to automate post-discharge care coordination. This functionality reduces preventable readmissions by ensuring timely follow-ups and allowing care teams to prioritize patients who need the most attention.

The EHR-agnostic Copilot and Agents seamlessly extend AI capabilities to customers’ existing EHR and care management systems, enabling natural language interactions and bridging multi-system workflows with intelligence. By working within existing infrastructure, Innovaccer ensures fast adoption and value realization without requiring large system overhauls.

“In healthcare, every minute a care manager spends on administrative tasks is a minute taken away from a patient. With Copilots and Agents, we’re changing that. This is about transforming care delivery, not by replacing people, but by empowering them with AI that works seamlessly within their current systems,” said Abhinav Shashank, cofounder and CEO, Innovaccer.

The launch aligns with Innovaccer’s mission to enable connected, proactive, and personalized care by unifying data, workflows, and AI. Over 96,000 providers already use Innovaccer’s platform to streamline operations and drive better outcomes.
